1. Information Collection and Use
Physicians Laboratories collects information from our users at several different points on our website including when you place an order or request free materials. Physicians Laboratories is the sole owner of the information collected on this site. We will not sell or rent your email address, phone number, address, or financial information to others. We request information from the user on our order form or free sample request forms. Here a user must provide contact information (name and shipping address) and financial information (credit card number, expiration date). This information is used for billing purposes and to fill customer's orders. If we have trouble processing an order, this contact information is used to get in touch with the user. You may receive order confirmation, information on product research and other emails from time to time. Users who no longer wish to receive our information may opt-out of receiving these communications by visiting our unsubscribe page. It may take several days to completely remove a user from all of our lists. If this privacy policy changes, the revised policy will be posted on this site. We may share sales information with the entity that referred you to Revival.
2. Security
This website takes every precaution to protect our users' information. When users submit sensitive information via the website, that information is protected both online and off-line. When our registration/order form asks users to enter sensitive information (such as credit card number and/or social security number), that information is encrypted and is protected with the best encryption software in the industry (SSL). While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user information off-line. All of our users' information, not just the sensitive information mentioned above, is restricted in our offices. Furthermore, ALL employees are kept up-to-date on our security and privacy practices. Our employees are notified and/or reminded about the importance we place on privacy, and what they can do to ensure our customers' information is protected. Finally, the servers that we store personally identifiable information on are kept in a secure environment.

A cookie is a piece of data stored on the user's hard drive containing information about the user. Usage of a cookie is in no way linked to any personally identifiable information while on our site. Cookies can also enable us to track and target the interests of our users to enhance their experience on our site. Some of our business partners use cookies on our site (for example, advertisers). 4. Log Files
We use IP addresses to analyze trends, administer the site, track user's use of the site's features, and gather broad demographic information for aggregate use. IP addresses are not linked to personally identifiable information. We may share aggregated demographic log file information with our partners and advertisers. This is not linked to any personal information that can identify any individual person. All product shipping and payment processing is done by Physicians Laboratories. No outside parties, other than the processing bank, have access to this information.

6. Newsletter
If a user wishes to subscribe to our newsletter, we ask for contact information such as name and email address. To unsubscribe from our newsletter, please click any Unsubscribe link at the bottom of every email we send. It may take several days to completely remove a user from all of our lists.
7. Surveys and Contests
From time-to-time our site requests information from users via surveys or contests. Participation in these surveys or contests is completely voluntary and the user therefore has a choice whether or not to disclose this information. Information requested may include contact information (such as name and shipping address), and demographic information (such as zip code, age level). Contact information will be used to notify the winners and award prizes. Survey information will be used for purposes of monitoring or improving the use and satisfaction of this site.
8. Tell-A-Friend
If a user elects to use our referral service for informing a friend about our site, or for requesting a free sample for a friend, we ask them for the friend's name and email address. Physicians Laboratories will automatically send the friend a one-time email inviting them to visit the site, or informing them that their free sample has been mailed. The friend may unsubscribe by visiting our unsubscribe page. It may take several days to completely remove a user from all of our lists.
